Separate Colonization From Disease

Candida commonly inhabits mucosa. Disease begins when local ecology, tissue injury, immune control, organism behavior, and symptoms align.

Move from commensal ecology to disease only when the tissue compartment and host response support it.

Start with ecology

Candida can be present without causing illness. A culture or molecular signal from the mouth does not establish a treatment indication without compatible lesions or symptoms.

Identify disrupted control

Advanced HIV, corticosteroids, antibiotics, diabetes, malignancy therapy, dentures, xerostomia, malnutrition, and mucosal injury can shift commensal organisms toward disease.

Separate compartments

Oropharyngeal and esophageal candidiasis are mucosal syndromes, not candidemia. Esophageal disease is deeper than oral disease and always requires systemic treatment.

Look for invasive danger

Fever, hemodynamic instability, bloodstream risk, central lines, neutropenia, organ lesions, or persistent sepsis requires a separate invasive-candidiasis pathway.

Recognize common forms

Pseudomembranous disease produces removable white plaques with an erythematous surface beneath. Erythematous disease can cause red, sore mucosa without prominent plaques. Angular cheilitis affects the mouth corners.

Protect the differential

Leukoplakia, oral hairy leukoplakia, lichen planus, aphthous or viral lesions, mucositis, debris, and malignancy can resemble Candida disease. A nonremovable or atypical lesion deserves reassessment.

Use testing selectively

Typical initial disease is often clinical. Potassium hydroxide microscopy, culture, species identification, or biopsy becomes more useful with atypical, recurrent, or refractory disease.

Audit recurrence

Review ART and CD4, inhaled corticosteroid technique and rinsing, denture hygiene, xerostomia, diabetes, antibiotics, nutrition, tobacco exposure, and medication adherence.

Painful or difficult swallowing in an immunocompromised host can indicate esophageal candidiasis even when oral plaques are absent.

Recognize the syndrome

Use an initial therapeutic trial

A systemic fluconazole trial is reasonable before endoscopy in a typical presentation. Most responsive patients improve within about 7 days.

Escalate nonresponse

If symptoms do not improve within 7 days, perform endoscopy with histopathology, culture, and speciation to identify resistant Candida, CMV, HSV, pill injury, reflux, malignancy, or another cause.

Protect intake

Severe pain can compromise hydration, nutrition, oral ART, and antifungal delivery. Select oral suspension or IV treatment and supportive care from actual swallowing ability.

Miconazole must adhere to the upper gum, clotrimazole must dissolve slowly, and nystatin must coat and remain against the mucosa. Schedule, dexterity, saliva, taste, and cognition affect success.

Escalate correctly

Moderate or severe disease, high immune risk, recurrence, esophageal symptoms, inability to use a local product, or local-treatment failure favors systemic therapy and renewed diagnosis.

Use systemic alternatives deliberately

Itraconazole solution, isavuconazole, voriconazole, posaconazole, lipid amphotericin B, or an echinocandin can serve selected alternative or refractory roles. Echinocandins have higher reported relapse than fluconazole.

Complete the course

Early symptom improvement does not replace the 14-to-21-day treatment duration. Track pain, swallowing, intake, weight, adverse effects, and the planned escalation date.

Fluconazole inhibits CYP2C9, CYP2C19, and CYP3A4 to varying degrees. Review warfarin, phenytoin, sulfonylureas, statins, calcineurin inhibitors, ART, and other narrow-therapeutic-index drugs.

Protect electrical safety

Fluconazole can prolong QT. Review baseline risk, potassium and magnesium, structural heart disease, and other QT-prolonging agents.

Monitor organ and immune reactions

Hepatotoxicity, rash, severe skin reactions, gastrointestinal effects, and hypersensitivity can occur. Stop and evaluate serious hepatic, mucosal, skin, or systemic findings.

Reserve suppression for recurrences

Frequent or severe recurrences can justify chronic fluconazole after individualized review. Reassess ongoing need, ART control, toxicity, interactions, adherence, and resistance risk regularly.
