Trace Desert Dust to Tissue Spherules

Coccidioides lives in arid soil. Airborne arthroconidia enter the lung and develop into tissue spherules that release endospores.

Map the geography

Endemic regions include much of Arizona, arid California, parts of New Mexico, Nevada, Utah, Texas, northern Mexico, and emerging areas such as eastern Washington. Travel history can matter years later.

Follow the form

Environmental hyphae fragment into arthroconidia that become airborne. In tissue, the organism forms large spherules containing endospores rather than budding yeasts.

Read host defense

Cell-mediated immunity usually contains infection. HIV, transplantation, TNF inhibition, corticosteroids, pregnancy, diabetes, and other conditions increase complicated disease risk.

Protect the laboratory

Culture can produce highly infectious arthroconidia. Alert the laboratory when Coccidioides is suspected so specimens and growth are handled with appropriate containment.

Recognize diffuse disease

Widespread infiltrates, hypoxemia, respiratory failure, hemodynamic instability, or rapid progression represents severe pulmonary disease and supports amphotericin induction.

Search by symptoms

Skin lesions, focal bone or joint pain, lymph-node disease, soft-tissue lesions, or neurologic symptoms direct targeted imaging and sampling. Routine whole-body imaging is unnecessary without clinical clues.

Protect the CNS

Persistent headache, nausea, cognitive change, cranial neuropathy, or gait change warrants lumbar puncture and CNS evaluation. Hydrocephalus can become a separate mechanical emergency.

Combine Serology With Site Evidence

Serology is central but not absolute. Timing, immune suppression, laboratory method, culture, pathology, imaging, and the suspected compartment determine diagnostic confidence.

Use serology thoughtfully

IgM can support early infection and IgG supports evolving or established infection. False-positive IgM and false-negative early or immunosuppressed results require clinical interpretation.

Use complement fixation longitudinally

Complement fixation titers can correlate with burden and support response monitoring, but no single titer proves or excludes dissemination. Compare trends with symptoms and imaging.

Obtain organism evidence

Respiratory, tissue, bone, skin, or CSF culture and pathology can establish site-specific disease. Notify the laboratory because cultured arthroconidia are hazardous.

Stage only where indicated

Use directed imaging or lumbar puncture when symptoms identify a possible extrapulmonary site. Routine lumbar puncture is not needed in the absence of neurologic symptoms.

Use an Oral Triazole for Focal Disease

Mild-to-moderate focal pulmonary disease in HIV is treated with an oral triazole. The choice depends on site, absorption, interactions, organ function, and follow-up.

Use fluconazole

Fluconazole 400 mg orally daily is appropriate for focal pulmonary infection. Review renal function, liver tests, QT risk, pregnancy, and interacting drugs.

Use itraconazole precisely

Give conventional itraconazole 200 mg three times daily for 3 days, then 200 mg twice daily. Verify formulation, food and acidity instructions, interactions, and a serum concentration after about 2 weeks.

Prefer itraconazole for bone or joint disease

Osteoarticular disease often favors itraconazole and requires prolonged treatment, imaging, tissue evidence, structural assessment, and surgical consultation when needed.

Define focal stopping

After 3 to 6 months, stopping can be considered with clinical response, CD4 at least 250, suppressed HIV RNA, and planned chest imaging and serologic monitoring.

Induce Severe Disease With Amphotericin

Diffuse pulmonary or severe nonmeningeal dissemination needs amphotericin until improvement, followed by prolonged oral triazole therapy.

Amphotericin B deoxycholate 0.7 to 1 mg/kg IV daily or a lipid formulation 3 to 5 mg/kg IV daily is used. A lipid formulation is favored when renal risk is important.

Prevent predictable toxicity

Monitor creatinine, potassium, magnesium, CBC, liver tests, infusion reactions, volume, and nephrotoxins. Formulations are not interchangeable milligram for milligram.

Transition after improvement

Continue amphotericin until meaningful clinical improvement, then use fluconazole 400 mg daily or itraconazole 200 mg twice daily with exposure verification.

Treat long enough

Diffuse pulmonary or nonmeningeal disseminated disease generally requires at least 12 months. Expert review, clinical response, serology, site recovery, ART, and immune status guide discontinuation.

Treat Meningitis as a Lifelong System

Coccidioidal meningitis differs from other fungal meningitides: high-dose oral fluconazole is preferred, IV amphotericin alone is ineffective, hydrocephalus may need shunting, and suppression is lifelong.

Use preferred therapy

Give fluconazole 800 to 1200 mg orally daily with specialist care. Alternatives include itraconazole, voriconazole, posaconazole, or isavuconazole in selected failures or intolerance.

Do not rely on IV amphotericin alone

Intravenous amphotericin alone is ineffective for coccidioidal meningitis. Refractory disease can require intrathecal amphotericin administered only by an experienced clinician.

Treat hydrocephalus mechanically

Headache, gait change, cognition, vomiting, visual symptoms, imaging, and pressure can reveal hydrocephalus. CSF shunting may be required in addition to antifungal therapy.

Continue lifelong treatment

Relapse after triazole discontinuation is very common. Continue suppressive treatment at therapeutic doses for life even after symptom control or immune recovery.

Manage fluconazole

Adjust maintenance exposure for renal function. Monitor liver tests, rash, QT risk, potassium, magnesium, and interactions with ART, warfarin, anticonvulsants, statins, and immunosuppressants.

Manage itraconazole

Document formulation and administration, obtain concentrations, and monitor liver tests, edema, heart-failure symptoms, potassium, CYP3A4 interactions, and strong inducers.

Manage amphotericin

Monitor renal function, electrolytes, CBC, infusion reactions, volume, and nephrotoxins throughout induction. A lipid product reduces but does not remove toxicity.

Manage continuity

Verify authorization, supply, swallowing, adherence, travel, monitoring access, and ownership. A preventable gap can reactivate controlled disease.

A person with HIV, newly positive Coccidioides IgM or IgG, no active-disease evidence, and CD4 below 250 can receive fluconazole 400 mg daily. First exclude focal, diffuse, disseminated, and CNS disease.

Restore immune control

Effective ART is central. Most patients should receive coordinated ART, but profound immunosuppression with disseminated or CNS disease can justify specialist-guided delay of 4 to 6 weeks and close monitoring.

Stop focal therapy carefully

For focal pneumonia, require 3 to 6 months of response, CD4 at least 250, suppressed HIV RNA, and continued chest imaging and serologic surveillance.

Respect syndrome-specific duration

Diffuse pulmonary or nonmeningeal dissemination generally needs at least 12 months and expert review. Meningitis therapy remains lifelong regardless of immune recovery.

Amphotericin B is preferred when treatment is required. Observation or an azole can be considered only through individualized risk discussion in selected cases.

Treat later disease

Fluconazole or itraconazole can be considered after the first trimester. Continue coordinated maternal-fetal, infectious-disease, and pharmacy monitoring.

Protect meningitis control

First-trimester meningitis management can require intrathecal amphotericin or another expert strategy while avoiding systemic azole exposure. Do not create an unprotected interruption in lifelong therapy.
