Build the Autoimmune Exocrinopathy
Sjogren disease is not a dehydration disorder. Immune mediated gland dysfunction creates dry eye and dry mouth, while B cell activation and systemic inflammation can affect nerves, lungs, kidneys, blood vessels, joints, and hematologic tissues.

Immune activity reduces lacrimal and salivary function and damages protective surfaces.
Anti-SSA, biopsy, and objective gland tests support classification but do not replace diagnosis.
Nerves, lungs, kidneys, vessels, joints, and blood can become clinically important targets.
Start with exocrine function
Lacrimal glands sustain the tear film, while major and minor salivary glands lubricate the mouth, protect enamel, buffer acids, support swallowing, and limit infection. Loss of output creates tissue injury as well as discomfort.
Build the immune model
T cells, B cells, autoantibodies, cytokine signaling, epithelial activation, and focal lymphocytic inflammation contribute to disease. Anti-SSA supports classification and pregnancy risk assessment, but no antibody result replaces the clinical phenotype.
Use current terminology
Many current sources use Sjogren disease to emphasize a systemic disorder. It can occur without another autoimmune disease or in association with conditions such as rheumatoid arthritis or systemic lupus erythematosus.
Protect seronegative disease
A negative standard antibody panel does not settle every case. Objective ocular, salivary, pathologic, and systemic findings remain important when clinical probability is high.
Separate cause from consequence
Dryness can also result from anticholinergic medicines, dehydration, diabetes, radiation, infection, environmental exposure, aging, and other diseases. A rigorous evaluation identifies mimics and complications before assigning cause.

Recognize the Full Clinical Spectrum
Persistent dry eye and dry mouth are central, but a complete assessment also looks for functional loss, tissue damage, systemic activity, and findings that cannot safely wait for routine follow-up.

Vision, speech, swallowing, sleep, nutrition, and dental health reveal the real cost of dryness.
Raynaud, purpura, neuropathy, dyspnea, renal loss, cytopenia, and gland swelling change the plan.
Eye pain, vision loss, progressive weakness, persistent gland enlargement, and B symptoms cannot wait.
Characterize ocular symptoms
Grittiness, burning, photophobia, fluctuating vision, contact lens intolerance, and reflex tearing can accompany aqueous deficiency. Eye pain, reduced vision, focal epithelial injury, or ulceration requires urgent ophthalmic evaluation.
Characterize oral symptoms
Patients may carry water, wake to drink, struggle with dry food, speak for shorter periods, lose taste, develop oral burning, or experience recurrent caries and candidiasis. Ask how symptoms change nutrition and medicine administration.
Search beyond the glands
Inflammatory joint symptoms, Raynaud phenomenon, purpura, neuropathy, interstitial lung disease, renal tubular disease, cytopenias, thyroid disease, lymphadenopathy, and salivary gland enlargement can occur.
Interpret fatigue carefully
Fatigue can reflect inflammatory disease, pain, poor sleep, depression, anemia, thyroid dysfunction, medicine burden, or another disorder. It deserves a differential diagnosis rather than automatic immunosuppression.
Name urgent findings
Severe eye pain or vision change, inability to maintain intake, rapidly progressive weakness, dyspnea, purpura, persistent gland swelling, unexplained fever, drenching night sweats, and substantial weight loss require prompt escalation.

Use Objective Tests Without Confusing Their Roles
The 2016 ACR EULAR criteria use five weighted objective findings. They improve consistency for research and support clinical reasoning, but a classification threshold is not a substitute for diagnosis or a reason to ignore early disease.

Either finding carries the greatest weight in the 2016 ACR EULAR score.
Ocular staining, Schirmer testing, and unstimulated saliva each add one when abnormal.
A score of four classifies an eligible patient, but early disease still requires longitudinal judgment.
Calculate the score
Anti-SSA positivity and focal lymphocytic sialadenitis with a focus score of at least one each contribute three points. Ocular staining score at least five, Schirmer at most five millimeters in five minutes, and unstimulated whole saliva flow at most 0.1 milliliters per minute each contribute one point. A total of four classifies an eligible patient.
Check applicability
The criteria apply to patients with compatible dryness or systemic suspicion after relevant exclusions are considered. Head and neck radiation, active hepatitis C, AIDS, sarcoidosis, amyloidosis, graft versus host disease, and IgG4 related disease can interfere with classification.
Distinguish classification from diagnosis
Classification criteria were designed to standardize research populations. A clinician may continue evaluation when probability remains high despite a score below four, especially early in disease.
Use each test for its question
Schirmer testing measures tear production, ocular staining measures surface injury, salivary flow measures gland output, serology identifies immune markers, and labial biopsy can demonstrate focal inflammation. None answers every question alone.
Review reversible interference
Anticholinergic medicines and other sicca causes should be addressed when feasible before testing. Biopsy interpretation requires experienced pathology because focus score and tissue quality matter.

Protect the Ocular Surface From the First Visit
Dry eye care begins with mechanism, severity, and tissue protection. Aqueous deficiency can coexist with meibomian gland dysfunction, so lubrication alone is not always a complete plan.

Review airflow, smoke, low humidity, prolonged screen use, incomplete blinking, contact lenses, and medicines that worsen dryness. Environmental changes support therapy but do not replace treatment of active surface disease.
Choose lubrication deliberately
Artificial tears are foundational. Preservative free products are preferred when drops are used frequently or preservatives cause irritation. Gels and ointments last longer and are often useful at night but can blur vision.
Treat mixed mechanism disease
Warm compresses, lid hygiene, and targeted management of meibomian gland dysfunction may be needed when evaporative disease accompanies aqueous deficiency.
Conserve tears selectively
Punctal occlusion can retain tears in selected patients. Surface inflammation, infection, tear composition, and the broader treatment plan should be assessed before plugs or cautery are used.
Escalate before sight is threatened
Persistent pain, photophobia, reduced vision, epithelial defect, or corneal ulceration warrants urgent examination. Severe disease may require autologous serum, scleral lenses, or other specialist procedures.

Treat Ocular Inflammation on the Correct Timeline
Topical cyclosporine and lifitegrast address inflammatory dry eye rather than providing immediate lubrication. Their value depends on regular use, correct technique, realistic timelines, and severity based ophthalmic follow-up.

Cyclosporine reduces ocular surface inflammation and can support tear production in selected patients. Burning can occur, and meaningful benefit may take weeks to months. Some emulsions require gentle inversion before use according to product instructions.
Use lifitegrast
Lifitegrast is an LFA-1 antagonist labeled for signs and symptoms of dry eye. Instill one drop in each eye twice daily about 12 hours apart. Irritation, dysgeusia, and transient visual effects are important counseling points.
Teach device and lens technique
Single dose containers are discarded after use. Contact lenses are removed before lifitegrast and may be reinserted after 15 minutes. Avoid touching the container tip to the eye or another surface.
Use corticosteroids narrowly
A short topical corticosteroid course can be useful in selected inflammatory flares, but intraocular pressure, cataract, infection, and corneal risks require eye specialist supervision. It is not an indefinite self-directed treatment.
Match advanced care to severity
Punctal procedures, autologous serum tears, scleral lenses, amniotic membrane, and other interventions are considered when optimized topical therapy does not adequately protect the ocular surface.

Saliva protects much more than comfort. A prevention plan begins before the first cavity and integrates fluoride, dental surveillance, salivary support, infection recognition, diet, and medicine review.

Stimulate safely
Frequent water, sugar free xylitol gum or lozenges, saliva substitutes, and humidification can reduce burden. Avoid using frequent sugar or acidic candy as a salivary strategy because it can accelerate caries and erosion.
Review medicines and products
Anticholinergic medicines, sedating antihistamines, some antidepressants, bladder medicines, tobacco, alcohol, and alcohol containing mouth products can worsen dryness or oral injury. Modify therapy only through coordinated risk benefit decisions.
Look for complications
Inspect for caries, erosion, cracked mucosa, ulcers, denture injury, candidiasis, altered taste, and swallowing difficulty. Oral burning or plaques should not automatically be labeled autoimmune.
Protect intake and administration
Difficulty swallowing dry food can reduce nutrition and adherence. Use moisture, texture modification, dietitian support, and dosage form review rather than crushing medicines without checking whether the formulation permits it.

Use Systemic Secretagogues as Measured Therapeutic Trials
Pilocarpine and cevimeline can increase exocrine secretion when residual gland function remains, but their systemic muscarinic effects require cardiopulmonary, ocular, gastrointestinal, and medication review.

Thirty milligrams three times daily can improve saliva with a distinct schedule and tolerability profile.

Use pilocarpine precisely
Current labeling recommends pilocarpine 5 mg by mouth four times daily for Sjogren related dry mouth. Sweating, flushing, urinary frequency, nausea, diarrhea, dizziness, visual change, bradycardia, and bronchial effects can occur.
Use cevimeline precisely
Cevimeline is labeled at 30 mg by mouth three times daily for Sjogren related dry mouth. It can increase salivation but also sweating, gastrointestinal effects, secretions, airway resistance, heart rate or conduction effects, and miosis.
Respect contraindications
Cevimeline is contraindicated in uncontrolled asthma, known hypersensitivity, and when miosis is undesirable, including acute iritis and angle closure glaucoma. Pilocarpine has parallel concern for uncontrolled asthma and conditions where miosis is undesirable.
Use caution beyond the label headline
Significant cardiovascular disease, controlled pulmonary disease, gallbladder or biliary disease, kidney stones, and medicines that alter cholinergic function or heart rate may require closer review.
Define trial success
Choose a functional target such as easier speech, safer swallowing of dry food, fewer nighttime awakenings, or improved oral comfort. Reassess response, sweating, hydration, hemodynamics, vision, breathing, and adherence rather than continuing an ineffective drug indefinitely.

Treat Active Organ Disease, Not Dryness by Default
Systemic treatment is organized around the affected organ, severity, reversibility, and risk. Topical care remains central for dryness, while glucocorticoids, conventional immunosuppressants, and biologics are reserved for selected systemic disease.

Hydroxychloroquine may be considered in selected inflammatory musculoskeletal or systemic contexts. It is not a dependable treatment for isolated dryness or fatigue, and standard retinal, interaction, and dose safety still apply.
Use glucocorticoids cautiously
When active systemic disease requires glucocorticoids, use the minimum effective exposure and develop a taper or steroid sparing strategy. Infection, bone, metabolic, ocular, cardiovascular, and adrenal harms remain important.
Match immune therapy to the organ
Conventional immunosuppressants and biologics are selected for manifestations such as vasculitis, significant lung disease, renal disease, inflammatory neurologic disease, or hematologic complications. Evidence and choice differ across organs.
Use biologics for selected severe disease
Rituximab and other targeted approaches may be considered for severe refractory systemic disease in specialist care, especially where B cell driven pathology is prominent. They are not routine therapy for uncomplicated sicca.

Sjogren disease carries a clinically important lymphoma risk. Surveillance is not indiscriminate scanning; it is disciplined attention to gland, node, spleen, skin, constitutional, laboratory, and disease activity changes.

Low complement, cryoglobulinemia, lymphopenia or other cytopenias, hypergammaglobulinemia, rheumatoid factor, monoclonal gammopathy, and clonal lymphocytosis can strengthen concern in context.
Separate B symptoms from chronic fatigue
Unexplained fever, drenching night sweats, and substantial unintended weight loss are not routine fatigue. Infection, malignancy, and active systemic disease require evaluation.
Investigate the change
The workup is directed by the finding and may include examination, blood counts, chemistry, complement, protein studies, imaging, ultrasound, flow cytometry, or tissue diagnosis through rheumatology, hematology, radiology, and pathology.
Avoid false reassurance
Routine symptoms may fluctuate, but persistent unilateral gland enlargement, hard or fixed nodes, progressive cytopenia, or a new monoclonal pattern requires an explicit plan rather than watchful silence.

Use guideline based surveillance
Current ACR guidance conditionally supports serial fetal echocardiography from weeks 16 through 26 for anti-SSA or anti-SSB positive pregnancies, with closer weekly surveillance after a prior affected infant. Hydroxychloroquine is conditionally recommended in antibody positive pregnancy unless contraindicated.
Review medicines as a system
Prescription, OTC, supplement, ocular, oral, and dental products all matter. Anticholinergic burden can worsen dryness, while immunotherapy, pregnancy, infection, organ function, and vaccine plans can alter safety.
Measure the disease domains
Follow symptom function, ocular staining and complications when indicated, dental injury, salivary function, nutrition, lung and neurologic symptoms, kidney and blood abnormalities, gland or node changes, and patient priorities.
Give a return plan
Earlier assessment is needed for vision change, severe eye pain, inability to eat or drink, new dyspnea, neurologic deficit, purpura, persistent gland swelling, fever, night sweats, weight loss, or medication toxicity.
