Recognize the Attack and Classify the Risk
Raynaud phenomenon is an episodic vascular response, not a synonym for every cold or blue digit. The first task is to establish the attack pattern and decide whether it looks primary, secondary, or like a different vascular problem.

Digital vasoconstriction reduces perfusion and may produce pallor, cyanosis, pain, or numbness.
Warmth can bring erythema, throbbing, swelling, and return of sensation.
Age, symmetry, tissue injury, examination, medicines, and systemic clues separate primary from secondary disease.
Recognize the sequence
Cold or emotional stress can produce sharply demarcated pallor, cyanosis, numbness, or pain. Rewarming may bring redness, swelling, and throbbing. A biphasic pattern can still be meaningful; a textbook triphasic sequence is not mandatory.
Identify a low-risk primary pattern
Primary Raynaud often begins at a younger age, affects both hands symmetrically, preserves pulses and tissue, and lacks abnormal nailfold or systemic findings. The diagnosis remains longitudinal because new features can change the probability.
Find secondary warning signs
Later onset, marked asymmetry, severe pain, ulcer, necrosis, abnormal nailfold capillaries, skin or joint findings, dysphagia, dyspnea, weakness, or a causal exposure should trigger focused evaluation.
Protect the differential
Acrocyanosis, pernio, erythromelalgia, embolism, thrombosis, vasculitis, fixed arterial disease, thoracic outlet disease, and neuropathic conditions can reproduce part of the story. Persistent color change or pulse loss is not a routine Raynaud attack.

Connect Vascular Tone to Tissue Injury
The visible color change reflects reduced digital perfusion, but the danger depends on whether reversible spasm occurs alone or alongside structural vascular disease.

Adrenergic and endothelial signals transiently constrict digital arteries and arterioles.
Structural injury, thrombosis, embolism, or atherosclerosis can reduce reserve beyond reversible spasm.
Sustained oxygen deficit can produce ulceration, infection, necrosis, and tissue loss.
Understand vasospasm
Cold and emotional stress can increase vasoconstrictor tone in digital arteries and arterioles. Neural and endothelial signals alter the threshold and duration of the response.
Interpret the colors
Pallor reflects low arterial inflow. Cyanosis can appear as deoxygenated blood remains in poorly perfused tissue. Erythema and throbbing can follow as flow returns.
Add structural disease
Systemic sclerosis and other secondary causes can add endothelial injury, luminal narrowing, thrombosis, inflammation, or macrovascular obstruction. Less vascular reserve means the same trigger can cause deeper ischemia.
Recognize threatened tissue
Persistent pallor or cyanosis, severe rest pain, ulceration, infection, sensory or motor change, and necrosis indicate inadequate tissue perfusion and require urgent evaluation.

Build a Focused Secondary-Disease Workup
Testing should answer a clinical question. History and examination determine when nailfold capillaroscopy, autoimmune testing, vascular imaging, or another focused study will change care.

Capture triggers, sequence, symmetry, duration, pain, ulcers, occupation, medicines, and systemic symptoms.
Assess pulses, skin, nails, capillary changes, joints, connective tissue findings, and vascular asymmetry.
Use nailfold capillaroscopy and targeted laboratory or vascular testing when the phenotype raises concern.
Take a complete history
Document onset, trigger, sequence, symmetry, duration, pain, photographs, ulcers, function, occupation, vibration, trauma, nicotine, prescribed and OTC medicines, recreational exposure, and family or autoimmune history.
Examine between attacks
Compare pulses and blood pressures, inspect skin, fingertips, nails and capillaries, and look for ulcers, scars, sclerodactyly, telangiectasia, joint or muscle findings, livedo, bruits, or neurologic deficits.
Use capillaroscopy and laboratories selectively
Abnormal nailfold morphology can support connective-tissue disease risk. ANA and targeted antibodies, blood counts, inflammatory markers, metabolic studies, or other tests should follow the suspected cause rather than form an indiscriminate panel.
Escalate vascular testing when indicated
Persistent unilateral symptoms, pulse deficits, trauma, embolic risk, severe pain, or nonhealing wounds can require Doppler studies, pressure measurements, angiographic imaging, or urgent vascular consultation.

Reduce Attacks Before Adding Burden
Thermal protection, exposure review, and a measurable self-management plan are foundational. They remain important after medication begins.

Layer clothing, protect hands and feet, prewarm environments, and rewarm gradually.
Address nicotine, stimulants, decongestants, occupational vibration, and avoidable medication triggers.
Track frequency, duration, pain, function, ulcers, and trigger context to judge whether the plan works.
Protect the whole body
Layer clothing, use insulated gloves and socks, prewarm cars and workspaces, avoid abrupt temperature transitions, and use gradual warm water or another safe method to restore flow. Excessive direct heat can burn numb tissue.
Remove avoidable vasoconstrictors
Address nicotine, pseudoephedrine and other decongestants, prescribed stimulants, ergot products, selected migraine medicines, recreational sympathomimetics, and unnecessary vascular triggers with the responsible clinician.
Modify occupational risk
Freezer work, cold water, vibrating tools, and repeated hand trauma can sustain attack burden. Protective equipment, scheduled warming, task modification, and occupational-health support may be required.
Measure response
Track attack frequency and duration, pain, function, photographs, ulcer development, and rescue behavior. A lower attack count does not represent success if pain or tissue injury worsens.

Use Dihydropyridine Therapy Deliberately
For systemic-sclerosis-associated Raynaud, current EULAR guidance places a dihydropyridine calcium antagonist, usually oral nifedipine, first. The useful dose is the one that improves perfusion without creating symptomatic hypotension.

Nifedipine is the usual first-line choice for systemic-sclerosis-associated Raynaud; amlodipine is another practical option.
Increase deliberately while monitoring attack burden, blood pressure, dizziness, headache, flushing, and edema.
Persistent ulcer, rest pain, or worsening ischemia requires escalation rather than routine dose cycling.
Choose a long-acting strategy
Long-acting nifedipine is the usual guideline-supported first-line oral option for systemic-sclerosis Raynaud. Amlodipine is another practical dihydropyridine when the clinical situation favors it.
Titrate to a clinical outcome
Increase therapy deliberately using attack frequency, duration, pain, function, and tissue status. Baseline blood pressure, orthostasis, cardiovascular context, and other vasodilators determine how much room exists.
Manage predictable toxicity
Headache, flushing, dizziness, peripheral edema, and hypotension can limit use. New near-syncope or falls should lead to dose and regimen reassessment rather than automatic escalation.
Know when oral therapy is not enough
A persistent ulcer, severe rest pain, infection, or failure to rewarm is not a reason to continue routine titration. It requires urgent specialist and vascular evaluation.

Assess pressure, headache, flushing, vision or hearing symptoms, interactions, and the reason for off-label use.
Use the mechanism
PDE5 inhibition reduces cyclic GMP breakdown and supports vascular smooth-muscle relaxation. EULAR recommends considering this class for systemic-sclerosis Raynaud when the patient and prior response support escalation.
Name the off-label decision
US sildenafil labeling covers specific approved indications, not Raynaud. Document the clinical rationale, expected outcome, dose plan, and evidence source rather than implying an FDA-approved Raynaud indication.
Protect the nitrate boundary
Sildenafil is contraindicated with organic nitrates and nitric oxide donors. The prohibition includes sublingual, oral, transdermal, and topical exposure because systemic hypotension can occur.
Review hemodynamic and interaction risk
Alpha blockers, antihypertensives, volume depletion, riociguat, and CYP3A4 inhibitors can change safety. Counsel about dizziness, syncope, headache, flushing, and urgent vision or hearing symptoms.

Treat a Focal Digit Without Losing the Systemic View
Topical nitroglycerin may support local perfusion, but the route does not eliminate systemic absorption, adverse effects, transfer risk, or the PDE5 inhibitor contraindication.

Topical nitroglycerin may target a severely affected digit, but absorption and headache remain clinically relevant.
A topical route does not remove the nitrate contraindication with sildenafil or another PDE5 inhibitor.
Clarify amount, location, timing, handwashing, blood-pressure symptoms, and what to do if pain worsens.
Specify the application
The prescription should define amount, digit or site, timing, maximum exposure, and whether a covering is used. Unmeasured application across several digits can increase systemic exposure.
Expect systemic effects
Headache, flushing, dizziness, and hypotension can occur. The response plan should include removal or withholding instructions, blood-pressure assessment, and a threshold for urgent evaluation.
Prevent unintended exposure
Teach handwashing or applicator use and how to avoid transfer to caregivers, children, eyes, or other skin. Product-specific storage and disposal instructions still apply.
Reconcile PDE5 therapy every time
Sildenafil and other PDE5 inhibitors must not be combined with nitroglycerin. A patient should not alternate the two without a clinician-directed plan that respects the relevant drug and label boundaries.

Recognize and Escalate a Threatened Digit
Persistent ischemia, rest pain, ulceration, infection, or neurologic loss can lead to irreversible tissue injury. This is an urgent syndrome, not a routine medication follow-up.

Persistent pallor or cyanosis, severe rest pain, ulcer, infection, necrosis, or neurologic loss requires urgent action.
Specialist care may use intravenous iloprost after oral therapy fails in severe systemic-sclerosis Raynaud.
Evaluate thrombosis, embolism, vasculitis, infection, macrovascular obstruction, trauma, and systemic disease.
Identify urgent findings
Failure to rewarm, persistent pallor or cyanosis, severe rest pain, a new or worsening ulcer, drainage, spreading erythema, sensory or motor change, and necrosis require urgent assessment.
Evaluate competing mechanisms
A threatened digit can reflect severe vasospasm, thrombosis, embolism, vasculitis, infection, trauma, or macrovascular obstruction. Pulses alone cannot exclude digital ischemia.
Escalate severe systemic-sclerosis Raynaud
Current EULAR guidance recommends considering intravenous iloprost when severe systemic-sclerosis Raynaud fails oral therapy. This is specialist-directed treatment with monitoring.
Treat the wound and circulation together
Assess perfusion, infection, pressure and trauma, wound depth, pain, and healing barriers. Antibiotics cannot restore perfusion, and vasodilation cannot substitute for infection control.

Treat the Cause Behind the Color Change
Raynaud can be the first visible sign of systemic sclerosis or another connective-tissue, vascular, hematologic, occupational, or medication-related process. Attack control does not end cause-directed care.

Organ screening and disease-specific care continue even when digital attacks improve.
Recognize connective-tissue context
Systemic sclerosis, lupus, mixed connective-tissue disease, inflammatory myopathy, Sjogren syndrome, and other disorders can alter vascular and organ risk. Skin, joint, muscle, pulmonary, gastrointestinal, renal, and constitutional findings matter.
Review medicines and substances
Stimulants, sympathomimetics, nicotine, ergot products, selected chemotherapy, and other vasoconstrictive exposures may provoke or worsen attacks. Assess indication and withdrawal risk before changing therapy.
Find occupational and vascular causes
Vibrating tools, cold exposure, repetitive trauma, thoracic outlet disease, atherosclerosis, embolism, thrombosis, and vasculitis require different prevention and treatment plans.
Continue systemic care
Improved finger color does not exclude pulmonary, renal, gastrointestinal, cardiac, or other connective-tissue disease involvement. The broader disease plan needs explicit ownership.

Make Episodic Disease Measurable
The treatment record should make each future decision easier. It must connect phenotype, triggers, tissue status, systemic context, exposure, response, toxicity, and escalation ownership.

Define the baseline
Record frequency, duration, pain, function, photographs, ulcers, blood pressure, occupational impact, trigger exposure, and secondary-disease findings before changing treatment.
Judge meaningful response
Look for fewer or shorter attacks, improved hand function, reduced pain, preserved tissue, wound healing, acceptable blood pressure, and tolerable adverse effects. Season and exposure can confound short comparisons.
Preserve the medication story
Document each agent, formulation, dose, dates, response, reason for discontinuation, adverse effect, nitrate or PDE5 boundary, and access barrier. A list without outcomes invites repeated failure.
Teach the urgent plan
Routine attacks should resolve with safe rewarming. Failure to rewarm, persistent severe pain, ulcer, drainage, weakness or numbness, infection, or necrosis requires urgent care and a named destination.
