Peripheral Arterial Disease

Treat PAD as systemic atherosclerosis

Lower-extremity plaque and thrombosis affect walking and tissue perfusion, but the same patient also faces myocardial infarction, stroke, and death. Major adverse cardiovascular events and major adverse limb events are parallel outcomes, so every plan needs both systemic prevention and limb care.

Recognize silent and atypical disease

Many people with objective PAD report no classic calf pain. They may walk too little to provoke symptoms, describe fatigue or weakness, or attribute limitation to age, arthritis, neuropathy, or lung disease. A normal symptom history does not erase abnormal physiology or functional impairment.

Separate claudication from limb threat

Claudication is reproducible exertional muscle discomfort relieved by rest. CLTI is chronic ischemic rest pain, nonhealing wound, or gangrene with objective arterial disease. ALI is a sudden perfusion decline, generally under two weeks, with pain, pallor, pulse loss, coolness, sensory loss, or weakness. These states have different time horizons and goals.

Include risk amplifiers and inequity

Diabetes, tobacco, kidney disease, polyvascular disease, age, depression, mobility loss, geography, structural racism, and access barriers influence detection, treatment, amputation, and survival. They are clinical data that should change screening, intensity, follow-up, and team design.

Physiologic Diagnosis and Anatomic Evaluation

Testing begins with a clinical question and progresses from resting physiology to exercise or tissue-perfusion studies, then to anatomy when intervention or uncertainty makes imaging useful.

Do not replace physiology by default

Measure the ABI correctly

Use Doppler systolic pressures in both arms and at the dorsalis pedis and posterior tibial arteries. Calculate each leg using the higher ankle pressure divided by the higher brachial pressure. Interpret the result with waveforms, technique, symptoms, diabetes, kidney disease, and edema.

Use exercise when rest is not enough

A fixed stenosis may preserve pressure at rest yet fail when muscle demand rises. When symptoms suggest PAD but the resting ABI is normal or borderline, standardized exercise with prompt postexercise ankle pressures can reproduce symptoms and expose a hemodynamic decline.

Work around calcified vessels

Diabetes and kidney disease can produce stiff, poorly compressible ankle arteries and a falsely high ABI. Toe-brachial index and digital waveforms often provide better physiology. In CLTI, toe pressure, transcutaneous oxygen, or skin-perfusion pressure can add information about local healing potential.

Image only when anatomy changes care

Duplex ultrasound, CT angiography, MR angiography, and catheter angiography answer different anatomic questions. Select the modality according to kidney function, contrast risk, calcification, devices, prior intervention, and whether revascularization is being planned.

Cardiovascular and Limb-Risk Pharmacotherapy

The medication plan protects several vascular beds at once and must distinguish single antiplatelet therapy, vascular-dose dual-pathway therapy, and full anticoagulation.

Choose antithrombotic intensity deliberately

Symptomatic PAD warrants single antiplatelet therapy with aspirin or clopidogrel. In selected symptomatic patients and after lower-extremity revascularization, rivaroxaban 2.5 mg twice daily plus low-dose aspirin reduces cardiovascular and limb events when bleeding risk is acceptable.

Do not confuse vascular dose with full anticoagulation

The PAD regimen uses a specific low rivaroxaban dose with aspirin. Full-intensity oral anticoagulation should not be used for PAD alone. If atrial fibrillation, venous thromboembolism, or another indication exists, define that indication and rationalize any accompanying antiplatelet therapy.

Treat PAD as clinical ASCVD

Use high-intensity statin therapy with a goal of at least 50 percent LDL reduction. If LDL remains at least 70 mg/dL on maximally tolerated statin, current guidance supports adding ezetimibe or a PCSK9 inhibitor. Pressure below 130 over 80 is the general target when tolerated.

Treat diabetes and tobacco exposure as vascular therapy

For type 2 diabetes, GLP-1 receptor agonists and SGLT2 inhibitors with cardiovascular benefit can reduce major events when appropriate. Tobacco treatment should combine repeated assessment, behavioral support, and pharmacotherapy rather than a one-time warning.

Prescribe exercise as treatment

Supervised exercise therapy and structured community programs with behavioral support improve walking performance, function, and quality of life. The prescription includes frequency, intensity, time, and type, with progression and reassessment. Unstructured advice to walk more is not an equivalent intervention.

Adapt the program without abandoning structure

Patients with balance problems, joint disease, amputation, or severe walking limitation may benefit from arm ergometry, recumbent stepping, or another structured nonwalking program. Foot status, cardiac safety, transport, schedule, footwear, and social support influence completion.

Use cilostazol only in an eligible phenotype

Cilostazol can improve claudication symptoms and walking distance. It should not be used in heart failure of any severity. Review pulse, palpitations, headache, bleeding therapy, CYP3A and CYP2C19 interactions, and whether a defined trial produces meaningful functional benefit.

Protect the foot before it becomes threatened

Inspect skin, callus, nails, deformity, sensation, footwear, temperature, and pulses. Teach daily self-inspection and early reporting. Neuropathy can remove pain as a warning, while poor perfusion reduces the capacity to heal a small injury.

Use a patient-centered indication

For functionally limiting claudication, revascularization is reasonable after an inadequate response to guideline therapy and structured exercise. If function and quality of life are acceptable, a procedure offers no established preventive benefit. For CLTI, the goal shifts to tissue healing, pain relief, and preservation of a functional limb.

Match approach to anatomy and patient reserve

Endovascular, surgical, and hybrid approaches differ in invasiveness, durability, conduit needs, wound burden, and repeat-intervention risk. Anatomy, life expectancy, frailty, kidney and cardiac risk, available vein, infection, patient preference, and local expertise belong in the choice.

Continue disease treatment after flow is restored

Revascularization does not cure atherosclerosis. Continue lipid, pressure, diabetes, tobacco, antithrombotic, exercise, and foot care. Document postprocedure antithrombotic purpose and duration so temporary combinations do not become accidental indefinite therapy.

Follow symptoms, function, and tissue

All patients need longitudinal examination and risk-factor follow-up. New symptoms after revascularization warrant ABI and arterial duplex assessment. A patent artery is not the only outcome: walking, wound healing, pain, mobility, recurrent events, and access to care matter.

Stage chronic limb threat

The WIfI framework organizes wound, ischemia, and foot infection. It helps estimate amputation risk and expected benefit from revascularization, but it does not replace anatomy, function, nutrition, comorbidity, goals, or local tissue expertise.

Assess acute viability at the bedside

In ALI, pain and pulse loss establish concern, while sensory deficit, motor weakness, and Doppler signals reveal increasing threat. Viability is time dependent. A threatened salvageable limb needs emergency reperfusion rather than avoidable diagnostic delay.

Coordinate reperfusion and systemic safety

Immediate vascular consultation, appropriate unfractionated heparin when not contraindicated, analgesia, and rapid endovascular or surgical planning are common elements. Embolic source, graft or stent thrombosis, bleeding, compartment syndrome, reperfusion injury, kidney risk, and systemic instability require parallel attention.

Define success as a functional recovery

Perfusion is necessary but not sufficient. Wound care, debridement when appropriate, infection management, pressure off-loading, nutrition, podiatry, rehabilitation, prosthetic planning when needed, cardiovascular prevention, housing, transport, and reliable follow-up determine whether the patient heals and remains mobile.
