Define the lesion precisely

A peptic ulcer is a mucosal defect that extends through the muscularis mucosae, most often in the stomach or proximal duodenum. An erosion is more superficial. Depth, location, morphology, and complications influence biopsy, healing, and follow-up decisions.

Protect a dynamic barrier

Surface mucus traps bicarbonate, tight junctions limit back-diffusion, mucosal blood flow removes acid and supports repair, and rapid epithelial restitution closes small injuries. Prostaglandins reinforce mucus, bicarbonate, perfusion, and cellular repair.

Connect cause to mechanism

H. pylori creates chronic inflammation and alters acid regulation. NSAIDs and aspirin reduce cyclooxygenase-derived prostaglandins. Smoking, severe physiologic stress, ischemia, hypersecretory states, and other uncommon processes can modify risk, but H. pylori and NSAID exposure remain the major causes.

Find bleeding early

Hematemesis, coffee-ground emesis, melena, syncope, orthostasis, tachycardia, iron-deficiency anemia, or an unexplained hemoglobin decline can indicate ulcer bleeding. Early hemoglobin may underestimate acute loss, so hemodynamics and the full presentation matter.

Escalate structural emergencies

Abrupt severe pain with guarding or a rigid abdomen suggests perforation. Persistent vomiting, early satiety, dehydration, and retained gastric contents can indicate gastric outlet obstruction. These presentations require immediate procedural or surgical evaluation, not outpatient medication escalation.

Respect gastric-ulcer follow-up

Gastric ulcer appearance, patient risk, biopsy findings, symptom course, and healing determine the need for repeat endoscopy. A persistent, atypical, or incompletely sampled lesion requires malignancy evaluation rather than reassurance from symptom improvement.

Audit every ulcer for H. pylori

H. pylori testing is central because eradication markedly reduces recurrence. A negative result is only useful when the test detects active infection and suppressive medications, acute bleeding, sampling, and prior treatment have been considered.

Reconstruct all ulcerogenic exposure

Ask specifically about ibuprofen, naproxen, aspirin, combination cold or pain products, intermittent high doses, multiple NSAIDs, corticosteroids, antiplatelets, anticoagulants, and selective serotonin reuptake inhibitors. The last three groups can amplify bleeding risk even when they do not create the ulcer through the same mechanism.

Do not accept an easy idiopathic label

If H. pylori and NSAID exposure appear absent, review test validity, gastric biopsy needs, malignancy, Crohn disease, ischemia, viral infection in immunocompromised patients, cocaine or other vascular injury, radiation, and gastrinoma or another hypersecretory state when the pattern supports it.

Use a test that answers the question

The urea breath test and validated stool antigen assay are preferred noninvasive tests for active infection. Endoscopic biopsy-based testing fits patients who already need endoscopy. Antibody tests can remain positive after cure and therefore do not establish active infection or prove eradication.

Protect test sensitivity

For proof of eradication, wait at least four weeks after completion of antibiotics. Hold bismuth and antibiotics for four weeks and PPIs or potassium-competitive acid blockers for two weeks when clinically safe. H2-receptor antagonists and antacids can be used as temporary symptom bridges when appropriate.

Plan cure testing at the first prescription

Every treated patient requires an active-infection test of cure. Schedule the test when therapy begins, provide written medication-hold instructions, and verify that the patient followed them before accepting a negative result.

Doxycycline is not the recommended substitute for tetracycline in this regimen.

Keep the fixed-dose product distinct

Pylera contains bismuth subcitrate potassium 140 mg, metronidazole 125 mg, and tetracycline 125 mg per capsule. Its current label directs three capsules four times daily after meals and at bedtime with omeprazole 20 mg twice daily for 10 days. This labeled pack is not identical to separately prescribed optimized 14-day therapy.

Engineer adherence

Use a written daily grid, count the total doses, review work and sleep schedules, separate tetracycline from antacids, iron, calcium, magnesium, and dairy, and have the patient repeat the plan. Counsel about nausea, metallic taste, photosensitivity, expected dark stool or tongue, neurologic symptoms, and signs of true bleeding.

Screen critical safety boundaries

Review pregnancy, renal function, disulfiram exposure, alcohol or propylene-glycol precautions, warfarin and lithium risk, other interactions, and current labeling. The combination may be effective only if the entire regimen can be taken safely and consistently.

The labeled Voquezna Dual Pak regimen is vonoprazan 20 mg twice daily with amoxicillin 1000 mg three times daily for 14 days.

Sequence salvage from prior therapy

For treatment-experienced patients who have not received optimized bismuth quadruple therapy, use optimized bismuth quadruple therapy for 14 days when appropriate. After prior optimized bismuth therapy, rifabutin triple therapy for 14 days is a preferred empiric salvage approach.

Reserve resistance-sensitive options

Do not use clarithromycin-containing or levofloxacin-containing salvage regimens without demonstrated susceptibility. Prior macrolide or fluoroquinolone exposure strengthens the reason to avoid empiric reuse.

Clarify penicillin allergy

A verified immediate penicillin allergy excludes amoxicillin-based options. When the history is vague or remote, formal allergy evaluation can safely recover effective choices. Never bypass a credible anaphylactic history merely to simplify the regimen.

List every previous macrolide, nitroimidazole, tetracycline, fluoroquinolone, rifamycin, and amoxicillin exposure. Use susceptibility data when available, avoid predictable resistance, and clarify penicillin allergy if it constrains effective therapy.

An uncomplicated duodenal ulcer commonly heals with about four weeks of PPI therapy, while gastric ulcers commonly receive about eight weeks and may require repeat endoscopy based on appearance, biopsy, risk, and response. Product-specific labeling and the clinical situation determine the final duration.

Use sucralfate precisely

Sucralfate adheres to the ulcer surface in an acidic environment. The labeled adult regimen for active duodenal ulcer is 1 g four times daily on an empty stomach for four to eight weeks, with antacids separated by at least 30 minutes. It can bind other medicines and aluminum can accumulate in kidney dysfunction.

Counsel about bismuth without missing bleeding

Bismuth can contribute antibacterial and mucosal-protective effects and commonly causes temporary darkening of stool or tongue. Black stool with weakness, dizziness, hematemesis, falling hemoglobin, or hemodynamic change must be treated as possible bleeding, not dismissed as a cosmetic effect.

Remove the driver

Acid suppression can close the lesion while H. pylori or NSAID exposure continues. The final plan must include eradication, NSAID revision, antithrombotic coordination, smoking support, malignancy evaluation when indicated, and a decision about maintenance protection.

NSAID management is a joint gastrointestinal and cardiovascular decision. The safest plan may be no NSAID, a lower-risk drug, or a protected regimen.

Use a non-NSAID strategy when reasonable. If an NSAID is necessary, select the lowest effective dose for the shortest duration, remove duplicate over-the-counter products, and consider gastrointestinal and cardiovascular tradeoffs. A COX-2-selective agent lowers gastrointestinal injury relative to many nonselective NSAIDs but does not remove all risk.

Use gastroprotection where benefit is meaningful

Daily PPI therapy is central for many high-risk patients who must continue NSAID or antiplatelet exposure. For the highest gastrointestinal risk and acceptable cardiovascular risk, a COX-2-selective NSAID plus a daily PPI can be appropriate. Protection must continue for the risk period.

Respect misoprostol's boxed warning

Misoprostol is labeled to reduce NSAID-induced gastric-ulcer risk in high-risk patients. The adult dose is 200 mcg four times daily with food for the duration of NSAID therapy, or 100 mcg if intolerance requires reduction. Diarrhea and cramping limit use. It is contraindicated for ulcer prevention in pregnancy and requires strict reproductive safety counseling.

Endoscopy is suggested within 24 hours after presentation once initial stabilization is underway. Active spurting, active oozing, and a nonbleeding visible vessel require endoscopic therapy. A clean-based ulcer does not receive the same endoscopic or post-procedure intensity.

Protect the clot after hemostasis

After successful endoscopic hemostasis, give high-dose PPI therapy continuously or intermittently for three days, followed by oral PPI twice daily through the first two weeks. This sequence reduces rebleeding risk while the lesion stabilizes.

Escalate recurrence and close the loop

Repeat endoscopy is suggested for recurrent bleeding. If endoscopic therapy fails, transcatheter arterial embolization is suggested. Confirm H. pylori eradication, avoid unnecessary NSAIDs, coordinate aspirin and antithrombotic resumption by indication and bleeding control, and continue long-term protection when recurrence risk remains high.
