Metronidazole, tinidazole, and secnidazole share a 5-nitroimidazole scaffold. Susceptible anaerobic and protozoal cells reduce the nitro group into reactive intermediates that damage DNA and kill the cell.

The shared scaffold predicts class effects

A prior serious reaction to one nitroimidazole raises concern across the class. Shared chemistry does not make agent exposure, approved uses, or counseling intervals identical.

Failure is not synonymous with resistance

Reduced uptake, increased efflux, and altered nitroreductase activity can reduce susceptibility, but adherence, reinfection, anatomy, and source control must be assessed first.

Oral metronidazole or tinidazole can treat selected Trichomonas, Giardia, and Entamoeba infections. Vaginal gel does not reach therapeutic levels in the urethra and perivaginal glands for trichomoniasis.

Source control determines success

Drain an abscess, debride infected tissue, or repair perforated anatomy when indicated. Longer antibiotic exposure cannot substitute for a missed procedure.

Current CDI care corrected older teaching

Metronidazole is not the preferred initial agent for most C. difficile disease. Use current syndrome-specific guidance, severity, recurrence, access, and patient factors.

Bacterial vaginosis is a polymicrobial vaginal dysbiosis. Trichomoniasis is a sexually transmitted protozoal infection. Their oral regimens overlap, but diagnosis, partner care, follow-up, and formulation choices do not.

CDC recommends metronidazole 500 mg orally twice daily for seven days for women. Vaginal gel is not effective treatment for trichomoniasis.

Men receive a different initial regimen

For men, CDC recommends metronidazole 2 grams orally once. Tinidazole 2 grams orally once is an alternative for women or men when appropriate.

Follow-up protects against reinfection

Treat sex partners, avoid sex until all treatment is complete and symptoms resolve, and retest sexually active women at about three months. Do not use NAAT before three weeks for persistent disease.

Secnidazole granules are not dissolved
