Define the stage
Natural menopause is confirmed after 12 consecutive months without menstruation when no other cause explains amenorrhea. Perimenopause begins before the final menstrual period and commonly includes cycle variability and vasomotor symptoms.
Recognize altered timelines
Menopause before age 45 is early, while loss of ovarian function before age 40 requires evaluation for primary ovarian insufficiency. Bilateral oophorectomy, pelvic radiation, and some systemic therapies can produce abrupt or treatment-related ovarian failure.
Know when to investigate
Typical symptoms and cycle changes in the expected age range are often diagnosed clinically. Pregnancy, thyroid disease, hyperprolactinemia, medication effects, abnormal uterine bleeding, and other causes should be considered when the pattern is atypical.

Characterize the burden
Record frequency, severity, night symptoms, sleep disruption, functional impact, duration, and triggers. Review fever, thyroid symptoms, infection, malignancy, medication effects, withdrawal, and other mimics when the presentation is not typical.
Choose a meaningful endpoint
Success is not necessarily zero symptoms. Define a patient-valued reduction in frequency, severity, sleep disruption, or functional impairment and use the same measure at follow-up.

Current progesterone labeling includes 200 mg at bedtime for 12 days per 28-day cycle with daily conjugated estrogens for endometrial protection. Fixed combinations and conjugated estrogens with bazedoxifene have their own doses, indications, and exclusions.

Bleeding after menopause, heavy bleeding, persistent bleeding beyond the expected adjustment period, or bleeding that begins after established amenorrhea requires evaluation according to gynecologic guidance. Hormone therapy should not be used to obscure an unexplained bleeding source.

Fezolinetant is an NK3 antagonist dosed 45 mg once daily. Current labeling carries a boxed warning for hepatotoxicity, requires baseline hepatic tests, and requires testing monthly for the first three months and again at months 6 and 9. CYP1A2 inhibitors are contraindicated.
Recognize the newer option
Elinzanetant is an NK1 and NK3 antagonist approved in 2025. The labeled regimen is 120 mg at bedtime, with product-specific dose modification and interaction guidance. Somnolence and driving safety should be reviewed using the current label.

Ospemifene 60 mg orally once daily with food is labeled for moderate to severe dyspareunia or vaginal dryness. It has estrogen agonist and antagonist effects and requires review of thromboembolic, bleeding, and cancer-related warnings and contraindications.

Transdermal estrogen may be preferred when avoiding first-pass hepatic exposure is important. Local therapy can be considered when symptoms are limited to GSM. A history of hormone-sensitive cancer requires nonhormonal options first and coordinated shared decision-making for local hormonal treatment.
