Map the Disease Biology
Ulcerative colitis and Crohn disease share immune-mediated intestinal inflammation but differ in distribution, depth, structural complications, and typical treatment response. The first clinical task is to name the phenotype accurately.

Recognize ulcerative colitis
Ulcerative colitis begins in the rectum and extends proximally through the colon in a continuous pattern. Inflammation is predominantly mucosal, so bleeding, urgency, tenesmus, and frequent stool are common.
Recognize Crohn disease
Crohn disease can involve any gastrointestinal segment from mouth to anus. Patchy transmural inflammation can create strictures, fistulas, abscesses, and perianal disease, while ileal disease can impair bile acid and vitamin B12 handling.
Describe behavior as well as location
Crohn disease can be inflammatory, stricturing, or penetrating, and those behaviors can coexist. Ulcerative colitis is described by proctitis, left-sided disease, or extensive colitis. These labels change delivery routes, prognosis, and procedural needs.
Look beyond the bowel
Arthritis, uveitis, skin disease, hepatobiliary disease, anemia, malnutrition, low bone density, growth effects, mental-health burden, and venous thromboembolism can shape treatment even when stool symptoms appear controlled.

Confirm Diagnosis and Measure Risk
No single symptom, antibody, or biomarker diagnoses inflammatory bowel disease. Confirmation integrates infection exclusion, ileocolonoscopy and biopsies, selective imaging, laboratory evidence, disease course, and future-risk features.

Exclude mimics before immunosuppression
At presentation, test for infectious etiologies that fit the syndrome, especially Clostridioides difficile in suspected ulcerative colitis. Medication injury, ischemia, microscopic colitis, celiac disease, infection, and functional disorders can produce overlapping symptoms.
Confirm anatomy and histology
Colonoscopy with ileal intubation and biopsies of affected and unaffected tissue confirms ulcerative colitis and helps distinguish Crohn disease. Small-bowel imaging is added when Crohn disease, proximal involvement, penetrating disease, or obstruction is suspected.
Use biomarkers as decision tools
Fecal calprotectin is sensitive to intestinal neutrophilic inflammation and can support diagnosis, response assessment, maintenance monitoring, and relapse evaluation. It is not disease specific and must be interpreted with the clinical context.
Measure future risk
Deep ulcers, extensive disease, young age at onset, prior hospitalization, steroid dependence, stricturing or penetrating behavior, perianal disease, smoking in Crohn disease, and prior surgery can justify earlier high-efficacy therapy.

Treat Beyond Symptom Relief
Modern inflammatory bowel disease care aims for durable steroid-free remission, normal daily function, biomarker improvement, and endoscopic improvement. Symptoms alone can understate persistent inflammation.

Restore daily life and resolve bleeding, urgency, pain, and fatigue.

Separate induction from maintenance
Induction controls active inflammation quickly. Maintenance preserves remission and prevents progression. Corticosteroids can induce remission but should not become maintenance therapy.
Use objective targets
Bleeding, stool frequency, urgency, pain, and function remain important, but CRP, fecal calprotectin, endoscopy, and selected imaging reveal residual inflammatory burden. Current ulcerative colitis guidance targets endoscopic improvement.
Reassess after a treatment change
Assess clinical response and objective inflammation after induction rather than renewing the same plan indefinitely. Persistent inflammation can reflect inadequate exposure, immunogenicity, nonadherence, structural disease, or mechanism failure.
Build preventive care into treatment
Review vaccinations, latent infections, nutrition, iron, vitamin B12, vitamin D, bone health, skin and cervical screening, colorectal surveillance, mental health, fertility, pregnancy plans, and tobacco exposure.

Treat Mild to Moderate Ulcerative Colitis
Disease extent directs local delivery. Rectal mesalamine is central for proctitis, combined oral and rectal therapy can improve left-sided disease, and inadequate 5-ASA response should lead to a new therapeutic class rather than endless product switching.

Treat the distal rectum directly.

Treat proctitis locally
A mesalamine suppository 1 gram daily is recommended for mild to moderate ulcerative proctitis. Rectal steroids are options when topical mesalamine is not tolerated or effective, while the exact formulation should match the inflamed segment.
Cover left-sided and extensive disease
Left-sided disease can use rectal mesalamine at least 1 gram daily plus oral mesalamine at least 2 grams daily. Extensive mild disease generally requires oral mesalamine, with rectal therapy added when distal symptoms persist.
Respect the dosage form
Mesalamine release technologies, strengths, instructions, and indications differ. Verify the exact product, assess renal function before and during treatment, support hydration, and counsel patients to report chest pain, worsening colitis, or hypersensitivity symptoms.
Escalate thoughtfully
After an adequate adherent 5-ASA trial, budesonide MMX 9 mg daily or a systemic corticosteroid can induce remission. Corticosteroids do not maintain remission, and switching to another 5-ASA formulation is not the preferred answer to a true class failure.

Select Advanced Therapy for Ulcerative Colitis
Moderate to severe ulcerative colitis has multiple effective mechanisms. Selection is a positioning decision involving prior exposure, efficacy, speed, route, age, comorbidity, reproductive goals, infection and cancer risk, and the patient’s priorities.

Use current options
Current recommended induction options include infliximab, adalimumab, golimumab, vedolizumab, ustekinumab, guselkumab, mirikizumab, risankizumab, tofacitinib, upadacitinib, ozanimod, and etrasimod. Choice is individualized, not alphabetical.
Do not require slow failure
For a patient with high inflammatory burden, steroid dependence, hospitalization risk, or impaired quality of life, an effective advanced therapy can be started without forcing failure of every older immunomodulator first.
Account for prior exposure
The best next treatment depends on whether a prior agent failed from inadequate exposure, antibodies, intolerance, or true mechanistic failure. Drug levels and antidrug antibodies can clarify selected cases of anti-TNF loss of response.
Plan maintenance at induction
Continue the effective advanced agent after response unless safety, intolerance, or loss of efficacy changes the plan. Infliximab plus a thiopurine has specific combination evidence, but combination therapy is not automatic for every drug.

Treat Crohn Disease by Risk and Behavior
Crohn disease treatment must address inflammatory activity and structural behavior separately. Budesonide can fit selected ileocecal disease, oral mesalamine is not an effective routine strategy, and fistula, abscess, or obstruction changes the care pathway.

Choose induction by location and risk
Ileal-release budesonide can induce selected mild to moderate ileocecal Crohn disease. Systemic corticosteroids can induce more extensive active disease but need an explicit steroid-sparing maintenance plan.
Retire ineffective mesalamine logic
Current ACG guidance recommends against oral mesalamine for induction or maintenance of mild to moderate Crohn disease. A familiar ulcerative colitis drug should not substitute for therapy that controls transmural Crohn inflammation.
Use advanced therapy early when needed
High-risk, moderate, severe, fistulizing, or rapidly progressive disease can justify early anti-TNF, vedolizumab, ustekinumab, IL-23, or upadacitinib therapy according to phenotype, evidence, and safety.
Treat structure and infection directly
Abscess requires drainage and antimicrobial assessment. Fibrotic obstruction may require endoscopic or surgical intervention. Perianal fistula care often combines imaging, drainage or seton, medicine, and colorectal surgical expertise.

Build Steroid-Free Maintenance
Maintenance therapy prevents relapse and structural progression without relying on corticosteroids. Thiopurines and methotrexate have slower onset, narrower roles, substantial monitoring, and patient-specific cancer, infection, liver, marrow, and reproductive risks.

Use thiopurines deliberately
Azathioprine and mercaptopurine are not rapid induction drugs. TPMT and NUDT15 assessment reduces severe myelotoxicity risk but does not replace CBC and liver monitoring. Pancreatitis, infection, lymphoma, and skin cancer remain important counseling topics.
Keep methotrexate disease specific
Parenteral methotrexate can support selected Crohn maintenance strategies. It is not recommended as monotherapy for ulcerative colitis induction or maintenance. Folate, CBC, liver, renal, pulmonary, interaction, alcohol, and pregnancy review are required.
Avoid steroid maintenance
Prednisone and budesonide cause cumulative glucocorticoid toxicity and do not provide an acceptable maintenance strategy. Recurrent courses, taper failure, or prolonged exposure should trigger reassessment of disease control and adrenal risk.
Reduce preventable harm
Before and during immunosuppression, review infection exposure, vaccines, dermatologic and cervical screening, blood counts, liver and kidney function, bone health, and signs of malignancy. Monitoring frequency follows the exact drug and phase of therapy.

Use gut-selective trafficking control
Vedolizumab blocks alpha-4-beta-7 integrin and limits gut lymphocyte trafficking. It is available through intravenous induction and intravenous or subcutaneous maintenance, with infection, vaccine, liver, and neurologic monitoring.
Distinguish interleukin targets
Ustekinumab blocks the shared p40 subunit of IL-12 and IL-23. Risankizumab, mirikizumab, and guselkumab selectively block IL-23 p19. Approved diseases, loading routes, dosing, and required monitoring differ by product.
Use current labeling
Risankizumab inflammatory bowel disease treatment includes tuberculosis evaluation, infection and vaccine review, intravenous induction, subcutaneous maintenance, and baseline plus induction-phase liver testing. Never transfer one biologic’s schedule to another.

Position JAK inhibitors
Tofacitinib is approved for ulcerative colitis and upadacitinib for ulcerative colitis and Crohn disease in defined patients. Boxed warnings include serious infections, mortality, malignancy, major cardiovascular events, and thrombosis. Age, smoking, cardiovascular, cancer, and thrombotic history shape selection.
Monitor JAK therapy
Screen for tuberculosis, hepatitis, infection, vaccines, pregnancy, blood counts, liver function, and lipids according to the product label. Avoid live vaccines and review CYP interactions. Induction and maintenance doses differ and should not be improvised.
Position S1P modulators
Ozanimod and etrasimod are ulcerative colitis options that retain lymphocytes in lymphoid tissue. Review conduction disease, heart rate, blood pressure, liver function, blood counts, macular edema risk, infection, vaccines, interacting drugs, and prior immune therapy.
Respect reproductive and interaction differences
Pregnancy avoidance periods, lactation data, CYP pathways, monoamine oxidase interactions, organ-function limits, and restart rules are product specific. A class summary never replaces the current prescribing information.

Recognize Emergencies and Structural Disease
Acute severe ulcerative colitis and complicated Crohn disease are multidisciplinary problems. Delay can convert reversible inflammation into perforation, sepsis, thrombosis, bowel loss, or death.

Use a day-three checkpoint
Current ACG guidance recommends infliximab or cyclosporine rescue when acute severe ulcerative colitis has not adequately responded after three days of intravenous corticosteroids. Prolonged ineffective steroid treatment should not delay surgery.
Avoid routine distractions
Routine broad-spectrum antibiotics without infection and parenteral nutrition solely for bowel rest do not improve acute severe ulcerative colitis. Antimotility and anticholinergic drugs can worsen dilation and are generally avoided.
Plan Crohn procedures and recurrence prevention
Abscess requires source control, fistulas need coordinated medical and surgical care, and fibrotic obstruction may require intervention. After resection, high-risk patients need preventive therapy and ileocolonoscopy in about 6 to 12 months because recurrence can precede symptoms.
