Build the Sweat-Control Circuit

Sweating is a protective output, not simply a nuisance. The hypothalamus, sympathetic cholinergic nerves, muscarinic receptors, eccrine coils, ducts, and evaporation form a control system that treatment must partially suppress without defeating thermoregulation.

Eccrine glands are widely distributed and central to cooling. Apocrine secretion contributes more to axillary odor biology and is not the main thermoregulatory target.

Trace the unusual sympathetic pathway

Postganglionic sympathetic fibers release acetylcholine onto muscarinic receptors at eccrine glands. This cholinergic terminal is an important exception to the common adrenergic pattern.

Connect sweat to cooling

Sweat lowers temperature only when it evaporates. Humidity, occlusive clothing, dehydration, and impaired airflow reduce cooling even when sweat production is high.

Distinguish trigger circuits

Heat and exercise recruit broad thermoregulatory sweating. Emotion can disproportionately affect palms, soles, axillae, and face. Trigger pattern helps map disease but does not prove its cause.

Recognize secondary disease

Endocrine disorders, infection, malignancy, neurologic disease, menopause, hypoglycemia, cardiopulmonary illness, substances, withdrawal, and medications can produce excessive sweating.

Reconcile the medication list

Antidepressants, opioids, hypoglycemic therapies, cholinergic drugs, and withdrawal states are examples. Timing around starts, dose changes, nonadherence, and discontinuation is more useful than a static list.

Measure what matters

The Hyperhidrosis Disease Severity Scale and patient-defined outcomes capture interference with work, school, grip, clothing, sleep, skin integrity, and social function. A single office observation can underestimate burden.

Antiperspirants reduce sweat output. Deodorants reduce or mask odor. Combination products may do both, but a deodorant-only product cannot substitute for antiperspirant pharmacology.

Apply to completely dry skin

Bedtime application allows longer contact during lower sweat output. Water can increase formation of irritating acidic species, so skin should be dry and intact.

Protect the barrier

Avoid application immediately after shaving or on broken skin. A thin, even film is preferable to repeated heavy application. Adjust frequency or formulation if dermatitis develops.

Move from induction to maintenance

Repeated bedtime use may be needed until control appears. Then reduce to the lowest frequency that maintains function, often once or twice weekly for some patients.

Apply one metered pump to each clean dry axilla at bedtime with the supplied applicator, allow five minutes to dry, avoid shaving for eight hours before, avoid washing for thirty minutes before and eight hours after, and wash hands.

Understand exposure differences

Sofpironium undergoes nonenzymatic hydrolysis and oxidative metabolism, including CYP2D6. Strong CYP2D6 inhibitors can increase exposure. Neither product belongs on broken skin, under occlusion, or on unapproved body sites.
