Gender-affirming care does not require one binary endpoint or one standard regimen. The person defines meaningful changes while the clinical team explains what hormones can and cannot control, what may be irreversible, and how the plan will be monitored.

Begin with the desired change

Ask which physical, sexual, reproductive, emotional, and social outcomes matter now. A person may want broad feminization or masculinization, a subset of effects, slower change, or no hormone treatment.

Explain coupled effects

Dose and route can influence pace and magnitude, but hormones do not reliably isolate one tissue. Voice deepening, facial hair, breast development, gonadal suppression, fertility change, and sexual effects differ in reversibility.

Use informed choice as a process

Discuss benefits, limits, alternatives, formulation technique, monitoring, fertility, contraception, uncertainty, and urgent symptoms. Revisit consent when goals, anatomy, health, access, or treatment change.

State the regulatory boundary

Common estradiol, testosterone, and adjunct products generally are not FDA approved specifically for gender affirmation in the United States. Off-label use can be evidence-informed, but the labeled indication, guideline evidence, product warnings, and monitoring should be distinguished clearly.

Avoid a laboratory-only endpoint

A concentration supports dosing and safety but cannot define identity or satisfaction. Clinical response, adverse effects, correct administration, assay timing, and the person's goals remain central.

Route youth care appropriately

Prepubertal and adolescent treatment involves development, consent, fertility, bone, family and social context, and local requirements. It belongs with an experienced multidisciplinary team rather than an adult regimen copied downward.

Stabilize rather than exclude

Address acute or unstable conditions and modify risk while preserving access whenever possible. A history of risk usually changes route, monitoring, or coordination rather than ending the discussion.

Discuss fertility before exposure

Estradiol, androgen suppression, testosterone, and gonadal surgery can reduce gametogenesis unpredictably. Offer timely information about sperm, oocyte, embryo, or tissue preservation without assuming a parenting goal.

Separate hormones from contraception

Amenorrhea and reduced sperm production do not prove infertility. Ask about retained organs and sexual practices, then offer pregnancy prevention and STI protection according to the person's preferences.

Order decision-linked tests

CBC is central before and during testosterone. Potassium and kidney function matter with spironolactone. Hormone, pregnancy, metabolic, and organ testing should answer specific regimen and health questions.

Use estradiol, not contraceptive ethinyl estradiol

WPATH recommends against ethinyl estradiol for feminizing therapy. It also advises against conjugated estrogens when estradiol is available because exposure is harder to monitor and thrombotic evidence is less favorable.

Compare routes, not just doses

Oral estradiol undergoes first-pass hepatic exposure. Transdermal delivery produces steadier systemic delivery and is preferred by WPATH for people older than 45 years or with previous VTE. Injectable products can create larger peak and trough variation.

Build a VTE strategy

Review prior VTE, thrombophilia, smoking, immobility, surgery, cancer, obesity, family history, and anticoagulation. Modify risk and coordinate complex histories rather than defaulting to unsafe exposure or automatic denial.

Interpret the concentration in time

Document route, exact product, dose, last administration, and collection time. Estradiol and testosterone concentrations are interpreted with physical response, adverse effects, adherence, and the assay.

Avoid supraphysiologic escalation

Slow breast or body-composition change can reflect normal biology. Increasing estradiol beyond physiologic targets does not guarantee more desired change and can add risk.

Keep spironolactone's full pharmacology

Its antiandrogen role coexists with mineralocorticoid receptor antagonism. Review potassium, kidney function, pressure, orthostasis, volume status, and other potassium-raising medicines.

Use GnRH therapy deliberately

GnRH analogues can suppress gonadal hormone production effectively but differ in onset and delivery and can be costly. Bone, fertility, gonadal status, dosing calendar, and access shape their use.

Set limits for 5-alpha reductase inhibition

Finasteride and dutasteride reduce conversion of testosterone to dihydrotestosterone. They may support a scalp-hair goal but do not provide broad testosterone suppression.

Do not add progestogens by reflex

SOC8 did not identify enough evidence to recommend routine progestogen use for feminizing therapy. A proposed breast, mood, sleep, or sexual goal should be balanced against uncertain benefit and product-specific risk.

Audit before stacking medicines

Persistent testosterone can reflect adherence, administration, timing, assay, dose, gonadal production, or interaction. Confirm the mechanism before adding another agent.
