Keep the Drug in the Lumen and the Mechanism at Transcription

Fidaxomicin is a macrocyclic antibacterial that binds bacterial RNA polymerase and inhibits RNA synthesis. It acts locally against C. difficile in the gastrointestinal tract and has minimal systemic absorption.

Correct the direct target

RNA synthesis is the direct molecular target. Reduced protein production is downstream, but fidaxomicin does not bind the bacterial ribosome.

The lumen is the intended compartment

Fidaxomicin and its main metabolite OP-1118 reach high fecal concentrations while plasma concentrations remain in the nanogram-per-milliliter range.

In vitro killing has boundaries

The drug is bactericidal against C. difficile in vitro and has a post-antibiotic effect. This does not prove eradication of colonization or systemic efficacy.

Resistance can alter the target

RNA polymerase changes can reduce susceptibility. Use the drug only for proven or strongly suspected CDI and preserve diagnostic stewardship.

A positive assay is not disease by itself

Asymptomatic colonization is common. Do not expose a patient without compatible illness to fidaxomicin solely because NAAT is positive.

Review the inciting exposure

Stop or narrow unnecessary non-CDI antibiotics when clinically safe. Continuing avoidable broad-spectrum therapy can impair response and promote recurrence.

Do not order a test of cure

After symptoms resolve, repeat testing is not recommended because colonization can persist despite successful treatment.
