Locate the pharmacophore

A large cyclic peptide core and lipophilic side chain support membrane association near the glucan synthase complex. This architecture contributes to potent target engagement and absent useful oral bioavailability.

Remove structural glucan

The Fks catalytic complex produces beta-1,3-D-glucan. Inhibition weakens the wall, alters shape and osmotic stability, and is generally fungicidal against susceptible Candida.

Separate wall from membrane

Echinocandins do not bind ergosterol and do not inhibit CYP51. Their mammalian selectivity follows from targeting a fungal wall component that human cells do not produce.

Read Aspergillus differently

The class damages growing hyphal tips but is not routine stand-alone initial treatment for invasive aspergillosis. Selected salvage or combination use requires syndrome-specific evidence.

Use the class for invasive Candida

Daily echinocandins remain recommended initial therapy for many adults with candidemia, especially with critical illness, prior azole exposure, or concern for azole resistance.

Interpret species carefully

Candida parapsilosis has intrinsically higher echinocandin MICs than many Candida species. Candida glabrata can acquire FKS resistance. Species identification and susceptibility guide refinement.

Treat Candida auris as two systems

Clinical infection generally receives initial echinocandin therapy with susceptibility and response monitoring. Colonization is not treated, but infection-control precautions and public-health coordination remain essential.

Respect limited compartments

Urine, eye, CNS, prostate, valve, bone, abscess, and infected hardware create exposure or source-control problems. Bloodstream activity cannot be generalized to every deep site.

Protect the adult loading dose

Most adult invasive indications use 70 mg once on day one followed by 50 mg daily. Esophageal candidiasis uses 50 mg daily without the studied loading regimen.

Adjust moderate hepatic impairment

Current labeling recommends 35 mg daily for adults with Child-Pugh 7 to 9 after a 70 mg load when appropriate. Severe hepatic impairment lacks a validated label pathway.

Respond to specified inducers

Rifampin uses 70 mg daily. A 70 mg daily dose should be considered with efavirenz, nevirapine, carbamazepine, dexamethasone, or phenytoin according to current labeling.

Keep dextrose out of the pathway

Caspofungin should not be mixed or diluted with dextrose-containing solutions. Use the exact labeled reconstitution, saline dilution, approximately one-hour infusion, and line-separation process.

Separate adult roles

Adult invasive candidiasis uses 100 mg daily, esophageal candidiasis uses 150 mg daily, and Candida prophylaxis in HSCT recipients uses 50 mg daily under current labeling.

Use age-specific pediatric logic

Children at least four months old use weight-based dosing with indication-specific maximums. Patients younger than four months use a distinct 4 mg/kg invasive-disease regimen when CNS or ocular dissemination is absent.

Do not overclaim neonatal CNS dosing

The effectiveness of a micafungin dose for Candida meningoencephalitis in young infants is not established. Higher exposure has limited data and requires specialist-guided interpretation.

Monitor the actual product

Hypersensitivity, hepatic, renal, hematologic, infusion, and injection-site effects remain visible. Micafungin can also increase sirolimus, itraconazole, or nifedipine exposure.

Load invasive candidiasis

Adults receive 200 mg once followed by 100 mg daily for candidemia and other invasive Candida infections. Esophageal candidiasis uses 100 mg once followed by 50 mg daily.

Follow nonenzymatic degradation

At physiologic conditions, the ring slowly opens into an inactive peptide that is further degraded. Human CYP enzymes do not drive its principal elimination pathway.

Avoid unsupported organ reductions

Current labeling requires no renal or hepatic dose adjustment, including dialysis. This pharmacokinetic rule does not remove liver, kidney, symptom, or infusion surveillance.

Calculate infusion time

The rate must not exceed 1.1 mg per minute. Labeled preparation yields about 45 minutes for 50 mg, 90 minutes for 100 mg, and 180 minutes for a 200 mg load.

Apply the current indication

Current 2026 labeling includes adults and patients age 12 years or older who have limited or no alternatives for candidemia and invasive candidiasis. It has not been studied for Candida endocarditis, osteomyelitis, or meningitis.

Protect the weekly regimen

Give 400 mg intravenously as the initial load, then 200 mg once weekly. Safety has not been established beyond four weekly doses, including the loading dose.

Recalculate after a delay

A dose given within three days of the assigned day preserves the next scheduled date. A delay beyond three days requires at least four days before the next dose. A gap of at least two weeks requires reloading.

Use product-specific surveillance

Hypersensitivity, infusion reactions, photosensitivity, liver abnormalities, neurologic symptoms, and gastrointestinal or electrolyte effects belong in the plan. Counsel on UV protection during treatment.

Do not borrow another recipe

Caspofungin excludes dextrose, while micafungin and anidulafungin have different compatible diluents and concentrations. Rezafungin has its own reconstitution and 250 mL infusion pathway.

Verify the complete preparation

Confirm product, dose, vial count, reconstitution volume, withdrawal volume, bag, final concentration, storage limit, line, co-infusion, pump, and duration before administration.

Classify histamine symptoms

Flushing, rash, pruritus, swelling, warmth, bronchospasm, hypotension, nausea, or chest tightness can occur. Slow or pause appropriate reactions, but do not normalize severe airway or circulatory findings.

Treat anaphylaxis immediately

Discontinue exposure and begin emergency evaluation and treatment for suspected anaphylaxis. A prior mild infusion reaction does not make later hypotension or wheeze routine.

Locate target resistance

FKS hotspot substitutions change the glucan synthase target and can predict clinical failure. Candida glabrata commonly involves FKS1 or FKS2, while Candida auris resistance often involves FKS1.

Interpret susceptibility in context

Species, method, current breakpoint, MIC, prior echinocandin, dosing, and clinical response all matter. Candida parapsilosis has higher baseline MICs that should not be mislabeled without context.

Audit persistent cultures

Reopen source control, catheter, abscess, site exposure, product, dose, preparation, host immunity, organism identification, susceptibility, and FKS testing before blind escalation.

Keep infection control active

Document bloodstream clearance

Repeat blood cultures daily or every other day establish the clearance date. Uncomplicated candidemia is generally treated for two weeks after clearance and symptom resolution.

Control the anatomic source

Remove or manage infected catheters and devices when appropriate, drain collections, debride necrotic tissue, and relieve obstruction. Persistent exposure cannot compensate for an uncontrolled focus.

Search when the course is not uncomplicated

Persistent cultures, focal symptoms, visual findings, murmur, hardware, neurologic change, bone pain, abdominal source, or urinary obstruction can require deeper evaluation and longer therapy.

Verify oral step-down
