Ear Anatomy and Symptom Triage
The canal, tympanic membrane, middle ear, inner ear, auditory nerve, mastoid, jaw, teeth, and throat can all generate an ear complaint. Localization protects time-sensitive hearing and neurologic pathways.

Localize before treating
Tragal tenderness and canal edema support external disease. Membrane findings, pressure, or effusion suggest middle-ear disease. Sudden hearing change, vertigo, or tinnitus can reflect inner-ear or neural disease, while jaw, dental, throat, or neck disease can refer pain.
Protect sudden hearing loss
Sudden sensorineural hearing loss is time sensitive. A patient reporting abrupt unilateral hearing loss, often with tinnitus or vertigo, needs urgent audiologic and otolaryngology evaluation rather than a prolonged trial of cerumen treatment.
Recognize invasive and neurologic danger
Severe persistent pain, mastoid swelling or tenderness, pinna displacement, cranial nerve deficit, systemic toxicity, trauma, foreign body, bleeding, severe headache, or high-risk immune status requires urgent evaluation matched to severity.
Use hearing as a functional vital sign
Clarify baseline, communication impact, device use, one versus both ears, fluctuation, and progression. A clear canal does not end the evaluation when hearing remains abnormal.

Acute Otitis Externa
Diffuse acute otitis externa is a painful canal infection commonly linked to moisture or trauma. Uncomplicated disease is treated topically, while pain and obstructed delivery are addressed directly.

Recognize canal infection
Rapid onset of ear pain, tragal or pinna tenderness, canal edema, erythema, and discharge supports diffuse acute otitis externa. Water exposure, cotton swabs, scratching, earbuds, hearing devices, dermatitis, and narrow canals can disrupt the protective barrier.
Use topical therapy first
AAO-HNS guidance recommends topical preparations as initial therapy for uncomplicated diffuse disease. Routine systemic antibiotics are reserved for extension beyond the canal or defined host and delivery factors. Choice depends on membrane status, allergy, formulation, organism, cost, and adherence.
Treat pain and blocked delivery
Assess pain severity and use suitable systemic analgesia. When edema or debris prevents drops from reaching the canal, trained aural toilet or wick placement can restore contact. Increasing drop volume alone does not solve a physical obstruction.
Reassess early nonresponse
Failure to improve within 48 to 72 hours prompts review of diagnosis, adherence, technique, canal patency, fungal disease, dermatitis, foreign body, perforation, resistant organism, and invasive extension.

Treat the membrane as a barrier decision
A tube, known perforation, trauma, surgery, or uncertain membrane changes which tissues contact the drug. Do not treat inability to see the membrane as proof that it is intact.
Avoid avoidable ototoxic exposure
When middle-ear exposure is possible, select a non-ototoxic preparation appropriate for that route. Aminoglycoside-containing products can create cochlear or vestibular risk when they cross a perforation unless specialist direction establishes a specific plan.
Recognize invasive external disease
Severe nocturnal pain, granulation, persistent discharge, cranial neuropathy, poor response, and diabetes or immune compromise raise concern for necrotizing external otitis and skull-base osteomyelitis. This is not routine swimmer's ear.
Escalate by host and depth
Assess glycemic control, immune status, prior radiation or surgery, hearing, balance, cranial nerves, fever, extension, and response. Urgent specialist evaluation and systemic management are required when invasive disease is suspected.

Use fluoroquinolones deliberately
Otic fluoroquinolone products provide local antibacterial activity, and selected formulations are suitable with tympanostomy tubes or perforation. Diagnosis and label still matter because not every product has the same indication, dose, vehicle, or age range.
Separate antimicrobial and anti-inflammatory roles
Combination ciprofloxacin and dexamethasone treats selected bacterial otic infections while dexamethasone reduces inflammation. Current labeling requires otic-only use, suspension shaking, exact dosing, and excludes viral and fungal canal infection.
Do not interchange eye and ear products blindly
Some ophthalmic solutions can be used in the ear under clinician direction, but otic products must never be placed in the eye. Sterility, isotonicity, pH, concentration, vehicle, and labeling differ. The route printed on the container must be verified.
Prepare the formulation correctly
Shake suspensions immediately before use. Warm the closed bottle in the hands for one to two minutes when the label directs to reduce dizziness from cold drops. Do not microwave, use hot water, contaminate the tip, or continue beyond the prescribed course.

Prepare before positioning
Wash hands, verify patient, product, route, affected ear, dose, expiration, seal, formulation, and storage. Shake and hand-warm only as directed. A written laterality plan prevents right and left errors.
Straighten the canal by age
Position the affected ear upward. Gently pull the pinna up and back for adults and older children and down and back for young children according to product instructions. Do not insert the dropper into the canal.
Instill and distribute
Place the exact number of drops without touching the tip. Gentle tragal pumping can help distribute solution when directed. A wick may need to be saturated according to clinician instruction. Avoid forceful pressure or instrumentation.
Maintain contact and protect the bottle
Remain with the treated ear upward for the label-directed interval. Recap without wiping the tip on a nonsterile surface. Complete the course, avoid sharing, and use return demonstration when dexterity or caregiver administration is uncertain.

Find the next diagnosis after wax clears
Treat clinical impaction, not visible wax
Symptoms can include hearing loss, fullness, pain, itch, tinnitus, or cough, and wax can prevent examination. Asymptomatic nonobstructive cerumen usually does not need removal because it protects and lubricates the canal.
Identify modifying factors first
Perforation, tube, ear surgery, canal stenosis, exostoses, dermatologic disease, anticoagulation, diabetes, immune compromise, hearing devices, and inability to report symptoms can change whether cerumenolytics, irrigation, manual removal, or referral is safest.
Use short evidence-based removal
Water, saline, mineral oil, hydrogen peroxide, carbamide peroxide, irrigation, and manual techniques may be used in selected patients. No one cerumenolytic is best for everyone. Follow the exact label and stop for pain, bleeding, discharge, dizziness, or hearing change.
Reject unsafe canal cleaning
Cotton swabs push wax deeper and can injure the canal or membrane. Ear candles can burn and do not provide reliable benefit. Probing, pressurized devices, and blind removal are unsafe. Persistent symptoms after clearance need another diagnosis.

Protect secondary pathways
Sudden hearing loss, focal neurologic symptoms, severe vertigo, trauma, unilateral persistent tinnitus, pulsatile tinnitus, asymmetric hearing, or concerning headache and visual symptoms can require urgent audiology, otolaryngology, vascular, or neurologic assessment.
Audit exposures without oversimplifying
Salicylates and other medicines can cause or worsen tinnitus in dose and host context. Review timing, kidney function, concentrations when relevant, interactions, noise, and alternatives. Do not stop an essential drug abruptly without a safe plan.
Treat burden rather than promising a cure
For persistent bothersome primary tinnitus, education, hearing evaluation, cognitive behavioral therapy, hearing aids when indicated, and selected sound strategies can improve function. Routine drug and supplement cures are not supported. Screen sleep, depression, anxiety, and suicidality.

Cerumen, hearing devices, canal skin, polypharmacy, tinnitus, cognition, falls, and age-related loss can coexist. Remove reversible obstruction, but obtain audiologic evaluation when hearing remains impaired because communication and cognition are clinical outcomes.
Lower the threshold in high-risk hosts
Diabetes, immune compromise, prior radiation, ear surgery, altered canal anatomy, anticoagulation, or recurrent severe disease changes the safety of home treatment and the urgency of persistent pain, discharge, granulation, or cranial findings.
Close the treatment loop
Set a 48 to 72 hour reassessment for treated acute otitis externa and a defined short course for cerumenolytics. Escalate worsening pain, fever, swelling, mastoid change, discharge, sudden hearing loss, vertigo, neurologic signs, bleeding, or failure to improve.
