Map Compensation, Pressure, and Decompensation

Cirrhosis is not one static state. Portal pressure and organ reserve create different trajectories and treatment windows.

Define the states

Ascites, overt encephalopathy, and portal hypertensive bleeding define decompensation. A prior event remains clinically important even after it resolves.

Explain portal pressure

Fixed fibrosis and dynamic vascular tone increase resistance. Splanchnic vasodilation and collateral formation then alter effective arterial volume and systemic circulation.

Connect the complications

Varices reflect collateral flow, ascites reflects sodium retention and pressure, HE reflects liver insufficiency and shunting, and HRS reflects severe circulatory and renal vasoconstriction.

Treat the cause in parallel

Viral cure, alcohol abstinence, metabolic treatment, autoimmune control, and removal of hepatotoxins can slow injury, but established complications still require active care.

Detect SBP

A PMN count at least 250 cells per cubic millimeter supports treatment even with a negative culture. Symptoms may be absent or limited to AKI or encephalopathy.

Search mixed etiologies

Cardiac disease, malignancy, tuberculosis, pancreatitis, thrombosis, and nephrotic disease can coexist with chronic liver disease and change the plan.

Weight-loss goals depend on edema and tolerance. Excess loss, hypotension, AKI, severe hyponatremia, potassium disturbance, cramps, or HE require reassessment.

Restrict water selectively

Routine fluid restriction is unnecessary for most ascites. Reserve it for clinically important dilutional hyponatremia according to current guidance and symptoms.

Do not chase ammonia

A normal ammonia value may prompt reconsideration, but serial levels do not reliably grade HE or direct dose escalation. Follow cognition, function, stool, hydration, and the precipitant.
