Restore Ventilation and Clearance

The paranasal sinuses drain through narrow ostia into the nasal cavity. Viral inflammation can obstruct those channels, slow ciliary transport, retain secretions, and create the setting in which a smaller fraction of illnesses become bacterial.

Begin with the sinonasal unit

Ciliated epithelium, mucus, ostia, and pressure regulation operate as one clearance system. Edema can produce pressure and drainage without bacterial infection.

Place the virus first

Most acute rhinosinusitis is viral. Neutrophils can make discharge opaque or colored, so appearance alone does not establish bacterial disease.

Name the usual bacteria

Streptococcus pneumoniae, Haemophilus influenzae, and Moraxella catarrhalis remain central pathogens when bacterial superinfection occurs.

Connect anatomy to risk

Allergy, smoke, structural obstruction, immune dysfunction, ciliary disease, dental infection, and prior procedures can change drainage, microbiology, or the need for specialist evaluation.

Classify the time course

Symptoms for at least 12 weeks require a chronic pathway with objective inflammation. Repeated discrete episodes with symptom-free intervals require a recurrent-disease evaluation.
