The paranasal sinuses drain through narrow ostia into the nasal cavity. Viral inflammation can obstruct those channels, slow ciliary transport, retain secretions, and create the setting in which a smaller fraction of illnesses become bacterial.

Protect Vision, Brain, Teeth, and Alternative Diagnoses
Routine sinus symptoms can overlap with orbital infection, intracranial spread, odontogenic disease, migraine, allergy, neuralgia, tumor, foreign body, and cerebrospinal fluid leak. Laterality and neurologic or eye findings matter.

Repeated facial pressure without a qualifying infectious trajectory may reflect migraine, allergy, medication effects, structural disease, tumor, or neuralgia.

Test Only to Answer a Clinical Question
Uncomplicated acute rhinosinusitis is a clinical diagnosis. Routine radiographs, CT, and nasopharyngeal cultures add noise. Testing is reserved for complications, alternative diagnoses, failure, recurrent disease, chronic disease, or surgical planning.

Make Observation an Active Treatment Plan
The 2025 adult guideline recommends offering watchful waiting for uncomplicated ABRS when follow-up is assured. Children with persistent illness may be observed for up to 3 days, while severe or worsening pediatric disease receives antibiotics.
